Please don't feel bad, this happens to many of us. I sometimes do not take a look at the date the thread was started before replying although that is a good habit to get into.

This is an old post and apparently no one had a template, so it looks like you'll have to make one up - please share if you come up with a good one!

One place to start is with the homeowners. We run an annual homeowner's survey (just released it earlier this month, since our budget process has started) and we ask the homeowners to rate the property manager's overall performance on a 1 - 5 scale, with 5 being the highest. There's also a space on the form where homeowners can comment about various issues, so if they have a specific tale of woe or high praise, they can write it down.

A review of the contract vs. the quality of the services received (and their costs) Does the property manager return calls in a timely manner? I would think emergency situations should be handled immediately, otherwise 24-72 hour turnaround might be sufficient. If the management company provides maintenance services, how well have they performed - was the work done according to specifications, are the prices competitive, were there any overcharges, were the homeowners happy?

Well since this is active again I guess I will make an attempt at providing something. This is what I have come up with for our 2010 HO survey. The BoD has approved the survey and it will be going out to HOs shortly.

I saved the doc in Word 97-2003 format for those of that dont have the newer version.

Any input will be welcome.

Your example is more like to be used for management evaluation by the board members. How about a list of more common and general questions that homeowners can easily make input? Thanks
